Modalités d’accès aux soins et gestion des motifs de consultation en territoire rural dans le Trièves : création, validation, diffusion et évaluation de supports d’information à destination des patients

Abstract : Introduction - Trieves is a mid-mountain territory in which, like in a lot of rural sectors, access to health services is a source of concern for inhabitants. They have a poor opinion of the existing access to health care, which could be related to a lack of knowledge about the health care facilities, and about the necessity and degree of urgency of different motives for consultation… The objective of this study is to evaluate the utility and pertinence for patients of the communication of information concerning the available health care and the management of motives for consultation. Method - This protocol consists of 4 steps. In the first instance, communication media were created, following a methodology guide for development of patient education materials, in the form of two posters and one brochure. These documents were presented to relevant health professionals, then to a sample of Trieves inhabitants, for validation and data collection, in a semi-structured interview during which health services accessibility was also discussed (textual analysis). These data were used to adapt documents, before their dissemination. Then they were assessed by patients through self-administered questionnaires. Results - 7 doctors, 29 health professionals and 13 Trieves inhabitants were interviewed. Different topics were discussed (health services accessibility, medical demography, medical permanence, etc), and submitted documents were validated. These were modified according to the remarks, then circulated. 69 patients answered self-questionnaires for final evaluation: despite certain limitations (non negligible rate of incomplete answers) they confirmed the utility and pertinence of these information supports with a 8.04/10 rate (CI95% [7,53;8,55]). Conclusion - Due to patient empowerment, health communication about concrete topics needs to be developed, particularly in rural areas where information can be hard to find. The utility and pertinence of this thesis enhances the patient’s knowledge, and it could be interesting to assess the impact of these documents on patients’ behaviour, and on health accessibility by a more appropriate recourse.
