Pratique du tiers payant intégral généralisé par les médecins généralistes en Aquitaine : une étude auprès de 12 médecins généralistes

Abstract : Background: the bill to introduce mandatory generalized third-party payment system was finally scrapped in 2017. A large number of general practitioners were opposed to its implementation. However, some already practice the generalized third-party payement system in a complete way, i.e. the total absence of advance payment for all their patients. Objective: to determine and describe the motivations, satisfactions and difficulties experienced by general practitioners practicing the generalized third-party payment system in Aquitaine. Method: qualitative study by semi-directed interviews with private general practitioners in Aquitaine practicing the integral generalized third-party payment system. After theoretical saturation of the data, an analysis of the transcribed verbatim revealed significant categories. Results: 12 interviews were conducted. Doctors practice the integral generalized third-party payment system mainly to facilitate access to care for all and to remove money from the consultation. Doctors say they are satisfied with the integral third-party payment system, which allows them to simplify the doctor-patient relationship for the sole purpose of care, as well as to lighten their accounting. The main difficulties with the full third-party payment system are its administrative burden and the difficulties of payment by mutual insurance companies. On the other hand, third-party payment does not encourage medical consumerism, the devaluation of the profession or dependence on paying organizations. Conclusion: because of its technical and administrative complexity, integral third-party payment system is difficult to generalize in its current state. The introduction of a single contact person would make integral third-party payment system acceptable to the rest of the general practitioners.
