État des lieux des pratiques de prescription de la NFS par les médecins généralistes : étude prospective en situation réelle auprès des médecins généralistes Maîtres de Stage des Universités de Bordeaux

Abstract : Introduction: a Full Blood Count (FBC) is the laboratory test which is the most frequently prescribed in general medecine. General Practitioners (GP) order three quarters of them. Its use should respect good working practice. There is no recent French study assessing the prescription of FBC by general practitioners. Aim: to describe the prescribing procedure of FBC by general practitioners. Method: the AquiNFS study is a descriptive prospective practice survey which was taken in the Aquitaine region (France) from May to July 2019. The population of our study consisted of University training mentors who prescribed an FBC to adult patients during their consultation. Junior doctors collected the data during their first ambulatory training with their mentors. Results: we collected 205 prescriptions. 61 is the average age of the patients who were prescribed an FBC and the majority of them were women (56.6%). Almost half of the prescriptions did not correspond to general recommendations (47.8%). The most common prescription indication were routine checkups (36.09%) and screenings for anemia (17.56%). We analysed 128 FBC among the 205 we had collected. One third was abnormal (38.2%). The most common pathology was anemia (40.8%). Then came leukocytosis (20.4%). Our study clearly shows that among the FBC which were prescribed without recommendations, there were three times fewer abnormal results (10.9 % vs 27.3%). When FBC is prescribed as a routine checkup, statistics show that the discovery of pathological results is less frequent. (p=0.01). Conclusion: one of the main results is the high rate of FBC prescriptions without recommendations, and more particularly as routine checkups which show low pathological results with minor disorders. These results are consistent with medical literature. To complete our study and to change medical practices, it would be interesting to explore the main factors leading to the prescription of FBC without recommendations. The method we used was a first approach to investigating primary care for junior doctors.
