Évaluation du risque de récidives de luxation antérieure de l’épaule chez le sportif après chirurgie pour instabilité de l’épaule

Abstract : Objectives: the main objective was to evaluate the risk of recurrence of anterior shoulder dislocation with athletes who underwent surgery for shoulder instability (coracoid bone block vs. capsulorrhaphy). The second objective was to compare the SIRSI score (psychological score estimating the ability to return to sport after shoulder pathology) according to the presence or absence of a recurrence of anterior shoulder dislocation following surgery. Material/Method: Monocentric retrospective study from 2012 to 2018 including competitive athletes or those practicing a sport profession who underwent surgery by coracoid bone block or capsulorrhaphy for shoulder instability and who benefited from a rehabilitation stay at the CERS for their shoulder pathology. The information was collected from a computerized database of the patient file including surgical, medical and sports data. Results: With an average 37 month delay of the response to the questionnaires, 171 patients met the inclusion criteria. 145 patients had undergone coracoid bone block surgery and 26 patients had undergone capsulorrhaphy. We found a significant difference (p=0.014) in the risk of recurrence according to the type of procedure; 6 patients (4.14%) in the coracoid bone block group had a recurrence compared with 5 (19.23%) in the capsulorrhaphy group. The SIRSI score was significantly lower (p=0.001) in patients who had a recurrence (33%) compared to those who did not (75%). Conclusion: The risk of recurrence of anterior shoulder dislocation in athletes is greater for capsulorrhaphy surgery than for coracoid bone block surgery. The SIRSI score is lower if the athlete suffers a recurrence following surgery, indicating a psychological fragility in his capacity to return to sport.
