Le rôle du médecin traitant dans l'éducation des enfants

Abstract

Introduction: Our society has undergone major changes over the past century, affecting the reference points that used to exist, particularly family and education. The latter, provided by parents, contributes to achieving good health, physical, mental, and social well-being. The general practitioner, a key figure in the monitoring of children and a central actor in health, is faced with this field where competence and empiricism seem to coexist. The main objective of this work is to determine whether the doctor has a role in the education of children, and if so, which one. Material and method: This is a qualitative study using semi-structured interviews, conducted with family doctors working in private practices in the Provence-Alpes-Côte d'Azur and Corsica French regions. Results: Thirteen general practitioners were interviewed between May and October 2020. The interviews highlighted the complexity of the educational task and the difficulty for the doctor to know how to deal with children, who are both full-fledged patients and under the responsibility of their parents. The lack of training in this area fosters a feeling of incompetence and uncertainty about the legitimacy of intervention in this field. However, the doctor's contribution is expected by education professionals, due to the unique position of first resort that he occupies and the relationship of trust he has with his patients. Added to this difficulty is the material constraint of an additional unpaid task for already overworked doctors. There is a fine line between health education and education. The latter concerns fundamental issues such as privacy, moral values and their transmission, as well as individual freedom and life in society, which is why it will always remain a delicate role. Conclusion: no attitude is neutral in terms of education and the doctor, faced with the demands of parents or educational shortcomings, has a role to play. Medical monitoring opens the door to educational support for parents to help them become aware of their responsibility and the need to set a framework. This already complex role is complicated by the lack of training to legitimise the doctor's remarks.
L’éducation est une préoccupation omniprésente dans la relation avec les enfants, y compris pour le médecin traitant qui les suit sur le plan médical en veillant à leur bonne santé. Être ou ne pas être co-éducateur, telle est la question. Le médecin généraliste a une position de premier recours facilement accessible lui donnant accès à un rôle pouvant être déterminant pour l’avenir des enfants. Son concours est attendu par les professionnels de l’éducation, du fait de la place unique qu’il occupe et la relation de confiance qu’il a avec ses patients. Son intervention peut paraître insignifiante pour certains médecins mais ne doit pas être sous-estimée car une seule rencontre peut marquer davantage qu’un suivi prolongé. Cette étude qualitative, par entretiens semi-dirigés auprès de treize médecins généralistes exerçant en libéral, a permis de mettre en évidence que le suivi médical ouvre effectivement la porte à un soutien éducatif. Les parents demeurent les premiers éducateurs et le soutien éducatif passe par leur accompagnement.
