Évaluation de la pertinence des radiographies anticipées dans les traumatismes de cheville chez les adultes aux urgences de Félix Guyon, La Réunion (étude RATAC)

Abstract : Introduction: Acute ankle injury is a frequent reason for visiting the emergency department (ED). An initiated X-rays protocol by the triage nurse (TN) has been officially in effect since 2019. This protocol allows the patient to reduce the time spent in the ED. The objective of this study was to evaluate if the initiated X-rays protocol by TN's leads to an over-prescription of X-rays performed in ankle injury compared to what would have prescribed by emergency physicians (EP's) without this protocol. Materials/Methods: This cross-sectional study included cases of adult patients between the ages of 18 and 55 who consulted between November 11th, 2020 and February 11th, 2021 in the ED of the hospital in Saint-Denis, Reunion island, for an isolated ankle injury. Results: Fifty patient cases were eligible; 46 cases were included. The percentage of X-rays wrongly requested by TN's was 44.4%. TN's requested 97.8% of X-rays, EP's 54.4%. In this study, there were 20 simple sprains (43.5%) and 11 severe sprains (23.9%) versus seven cases of fracture (15.2%). For non-severe diagnoses, EP's with more than 5 years' experience would have prescribed 57.1% of X-rays versus 28.6% for residents. Conclusion: The current initiated X-rays protocol shows a high percentage of unnecessary X-rays in ankle injuries. This work suggests the improvement of this protocol in order to reduce the over-prescription of X-rays.
