État des lieux des connaissances des internes de médecine générale de l’Océan Indien sur le diagnostic et la prise en charge des lésions cutanées mammaires

Abstract : Background: The breast and nipple can be the site of skin lesions of infectious, inflammatory and/or tumorous origin. This part of the body is of functional, aesthetic, psychological, and sexual significance. The first contact of a patient in their primary care journey is often with their general practitioner. The objective of this study was to assess the level of knowledge of interns in General Medicine (IGM) trained in Réunion and Mayotte in this field. Method: We conducted a quantitative, descriptive, and cross-sectional study with 252 IGMs from the Indian Ocean subdivision during their residency from October 2019 to April 2020. Interns were asked to complete a questionnaire consisting of 12 questions regarding their background as well as 10 clinical cases. Results: We received 105 responses. The average score was 47.5/100. Analysis by clinical case showed that participants had greater difficulties in identifying and treating dermatological and/or benign conditions. The scores for gynecological and/or tumor damage were higher. The power of the study was not high enough to highlight any significant difference in grades depending on university background and self-training. Conclusion: The results showed a knowledge gap in dermatology, specifically in the treatment of benign lesions. Changes in the theoretical curriculum of the 3rd Cycle could be considered (e-learning). More residency sites for dermatology as well as more comprehensive training for ambulatory medicine could help the IGMs improve their command of this type of pathology.
