Modifications du microbiote digestif de souris infectées par Helicobacter pylori en réponse à la metformine

Abstract : Metformin is widely prescribed to treat type 2 diabetes. This drug knows a renewed interest because epidemiological studies have shown that diabetic patients treated with metformin have a decreased risk of cancers, including gastric cancer. Metformin has also a direct antibacterial activity against Helicobacter pylori, which plays a crucial role in gastric carcinogenesis. Furthermore, recent studies show that the digestive microbiota can influence carcinogenesis. The aim of this work was to determine whether metformin alters the digestive microbiota and whether these changes promote pro-carcinogenic bacterial metabolic pathways, in the context of H. pylori infection. Mice were infected with H. pylori and treated for twelve days with either metformin or phosphate-buffered saline (PBS) as a control. At the end of the treatment period, the mice were sacrificed: stools, cecal and intestinal contents were collected. The gut microbiota of the three different digestive sites were characterized through 16S rRNA gene sequencing. In mice infected with H. pylori, metformin significantly decreased alpha diversity indices and led to significant variations in the relative abundance of some bacterial taxa including Clostridium sp. and Lactobacillus sp., which were directly inhibited by metformin in vitro. PICRUSt analysis suggested that metformin modifies functional pathway expression, including a decrease in nitrate reducing bacteria in the intestine. Metformin significantly changes the composition and predicted function of the gut microbiota of mice infected with H. pylori; these modifications could be implicated in digestive cancer prevention.
