Agressions armées en Guadeloupe : étude descriptive et rétrospective, de juillet 2017 à juin 2019

Abstract : Background : After French Guiana, Guadeloupe is the DOM with the highest incidence of armed attacks. Knowledge of its epidemiology is necessary to improve the medical care of these patients. Methods : This retrospective observational study, conducted in Guadeloupe, included all victims with stab or firearm injuries examined at the UML of teaching hospital, between 1 July 2017 and 30 June 2019. Suicides were excluded. The data are in absolute, percentage or median values, compared by the χ2 test and the Fisher exact test, and then a multivariate analysis is performed to look for factors predictive of mortality. Typical profiles with common characteristics are determined by a "latent class analysis". Results : Over the 2 years of the study, 404 victims were counted; 101 excluded due to lack of data, and 304 analysed. The majority were men (sex ratio M/F 8.5), young people (median age 31.5), stabbing victims (n=191) attacked in a delinquent context (70%), in the evening or at night (68%). The ratio of firearms to stabbing is 1:1.2. The mortality rate is 10% (n=30). The majority of victims die from firearms (p<0.001) following a thoracic lesion (p<0.001). We distinguish two typical profiles: a class of individuals in active employment, victims of robbery on the public highway, presenting little ballistic trauma. Profile 2: victims of delinquency or settling of scores, in a precarious social context (unemployed or temporary), who present more serious injuries caused by a stab weapon. Discussion : Armed assaults in Guadeloupe involve young men attacked in the evening or at night, mostly with knifes or machetes. Death occurs quickly, mostly caused by a thoracic lesion due to firearm. We identify two types of victims : the "robbed" type presents less severe injuries despite the use of firearms in the majority of cases, and the delinquency victim type; assaulted with weapons, presenting severe injuries due to penetrating thoracic trauma. Prevention and the fight against arms trafficking are two main lines of action to avoid these situations.
