Repérage par les professionnels de santé de la souffrance des proches aidants de patients atteints de cancer lors de la phase palliative avancée à domicile

Abstract : Background: with the increasing incidence of cancer and the French people’s wish to die “at home”, health caregivers are facing an increasing number of end-of-life care at home. The informal caregiver often presents signs of suffering that can make it difficult to stay at home. The aim of the study was to explore how professionals perceive these signals of suffering during the advanced palliative phase at home. Method : a qualitative study was carried out by focus groups on the places of practice of health professionals intervening at home during this late phase. The themes explored were spotting, evaluating, and sharing the suffering signals. Results : 4 interviews were carried out with groups of 4 to 11 health caregivers. Professionals perceive many signs of psychological suffering in informal caregivers instinctively and subjectively. The vulnerability factors of relatives identified by health caregivers are isolation, financial insecurity and psycho-cognitive disorders. Oral exchanges between professionals on this subject are rich and complicated by the number of speakers, with few written transmissions. Discussion : he predominance of the informal caregiver's psychological distress signals perceived by health professionals is linked to their emotional impact. The objectification of this identification requires a better knowledge of the psychological processes during this particular period. Anticipation of crisis situations at home also involves the early integration of palliative care when announcing a prognosis of non-curable, in an adjusted manner to each patient and each informal caregiver, as well as the valuation of the time spent to listen as a supportive care.
