Évaluation du langage oral chez les enfants de 5 à 6 ans 11 mois dans le cadre du trouble de stress post traumatique

Abstract : The effects of exposure to traumatic events on children and young children have long been underestimated. The consequences of post-traumatic stress disorder (PTSD) have mainly been studied from a psychiatric point of view; they therefore remain poorly documented with respect to language and communication development. We hypothesize that post-traumatic stress disorder in children causes oral language difficulties. Using calibrated oral language tests, we assessed the different components of oral language in 6 children, currently aged between 5 and 6 years 11 months, who developed PTSD following the attack on the Promenade des Anglais in Nice. We then compared each child's scores on each test with their risk factors. Our results showed pathological scores for 2 children, particularly in receptive lexicon (-3, 07σ and -2.45σ), expressive lexicon (-0.95σ and -1.96 σ) and morphosyntactic production (< -1 σ). In the norm for 3 children but with 1 child in the at-risk zone in receptive lexicon (-1.09 σ) and then 1 last slightly above average in expressive lexicon (> + 1 σ) but in the average for the rest of the tests. We also find a correlation between the presence or not of risk factors for each child and their scores on the tests performed. PTSD thus seems to have an impact on the appearance of difficulties in oral language, more particularly in the development of lexicon and syntax. The risk factors concerning the child's sex, the level of maternal education and prematurity are verified in children with oral language difficulties following a PTSD.
