Étude de la validité interne du questionnaire de repérage des difficultés d’adaptation à la pratique intensive du sport chez l’adolescent de classe sportive (DAPIS Ado)

Abstract : INTRODUCTION: High-level athletic teenagers face a double issue. Indeed, adolescence itself represents a pivotal period at risk of developing psychopathological disorders, to which are added the specifics constraints related to the practice of high-level sports. The legal framework regulates the supervision of these young athletes, with a regular physical and psychological assessment. This second feature is often overlooked. A multidimensional screening tool for psychopathological disorders has been created specifically for the teenage athletes and for an ease to use in general medicin: the DAPIS Ado questionnaire (Adaptation’s Disorders to the Intensive Practice of Sport in athletic Teenagers), divided into five sub-dimensions (psychological disorders, eating disorders, overtraining, social relationships, and substance use). This tool has not undergone a validation process yet. The aim of this work is to study the internal validity of this questionnaire. METHODS: This is an observational analytical cohort study. The participants are 8th to 12th grade students from the Jean Prevost school complex in Villard de Lans, practicing an individual sport. The data collection is prospective, via an online questionnaire on the secured platform Sphinx, repeated twice through two sessions spaced one month apart. The included subjects answered alone to the 49 items making up the DAPIS ado questionnaire, via an analog scale ranging from 1 to 10. The main purpose of our work is to study the internal validity of this questionnaire, with the evaluation of the internal consistency of each sub-dimension, which is assessed by Cronbach's alpha coefficient (alpha> 0.70 is satisfactory) and the evaluation of the reproductibility over time with the test-retest reliability (intraclass correlation coefficient > 0.75 is satisfactory). RESULTS: A satisfactory level of internal consistency was found for the “psychological disorders”, “eating disorders” and “overtraining” sub-dimensions, with Cronbach’s alpha coefficients respectively at 0.89, 0.85 and 0.75. However, the internal consistency for "social relations" and "substance use" dimensions wasn’t satisfactory with Cronbach’s coefficients respectively at 0.55 and 0.39. The reproducibility over time has a test-retest reliability highly satisfying with an intraclass correlation coefficient at 0.92 for the whole questionnaire. DISCUSSION: We identified several limitations to our study. The latter lacks statistical power because it was carried out on a small sample of 59 teenagers. Our sample corresponds to only 20% of the source population, with a participation on a voluntary basis, which creates a selection bias. The process of data collection by a self-questionnaire leads to a reporting bias, identified in the substance consumption dimension. Our results must be interpreted with caution because the statistical tool of the Cronbach’s Alpha coefficient seems to be influenced by many factors and therefore not very stable. This work enabled us to suggest argumentative modifications to the questionnaire. CONCLUSION: We carried out a first validation work on the DAPIS Ado questionnaire. While the reproducibility and feasibility of the questionnaire are excellent, internal consistency could not be confirmed in all dimensions, necessitating further studies on the subject, possibly with a more suitable statistical tool, and focusing on a largest sampling of teenagers.
