Évaluation des nouvelles pratiques de prise en charge des patientes présentant une rupture prématurée des membranes à terme à la maternité de l’Hôpital Couple Enfant : étude de type « avant / après »

Abstract : Objective: Evaluation of new management practices for patients with pre-labour rupture of membranes (PROM) at term in the maternity ward of the Hôpital Couple Enfant in Grenoble. Materials and methods: Retrospective before/after study comparing the old protocol with new service practices extending the expectant period before inducing labour after a PROM at terme. Four groups were formed according to the waiting period (old vs. new protocol) and carriage of group B streptococcus. Outcomes measured were spontaneous labour rate, mode of labour induction, route of delivery and maternal-fetal infection rate. Results: We included a total of 205 patients. For women with a negative or unknown group B streptococcus carriage, prolonged expectoration was associated with an non-significant increase of spontaneous labour. Regardless group B streptococcus carriage, no significant differences were found concerning the ceasarean section rate and the maternal-fetal infection rate. Conclusion: The outcomes appear to be comparable between the old protocol and the new service practices regarding the route of delivery and the risk of infection.
