Gliome diffus traité par radiothérapie post-opératoire : intérêt d’une rééducation spécifique de l’attention

Abstract : Attentional deficits induced by neurotoxicity of radiotherapy are recognized in adult patients with diffuse gliomas. Attention is necessary for the proper functioning of other cognitive skills and in our daily lives. Therefore, cognitive rehabilitation of this function seems relevant. Many publications concerning the retraining of attention disorders have shown the effectiveness of a specific approach proposing an intervention targeting the components that are specifically disturbed from those preserved. Using the Single Case Experimental Design methodology, we aimed to evaluate the effects of specific attentional rehabilitation in four adult patients with diffuse glioma treated with postoperative radiotherapy.
Measurements were repeated over a period of ten weeks, first during the baseline phase to determine the attentional component with the most important deficit, then during the rehabilitation phase. Error and omission rate, reaction time, working memory and fluency were assessed. The “Rating Scale of Attention Behavior” for complaints in everyday life was submitted at the beginning and the end of the study. Appropriate tools were chosen to meet the constraints of online assessment and attentional telerehabilitation.
Suggestive effects in accuracy were found for two patients but only for one at reaction times. Generalization in working memory seems limited and the specificity of rehabilitation could not be shown. However, an interesting transfer appears in everyday life. These encouraging findings show the relevance of post-radiotherapy attentional rehabilitation for these patients.
