Impact de l’âge gestationnel sur la survenue et le type de lésions cérébrales détectées à l’IRM chez des nouveau-nés atteints d’encéphalopathie anoxo-ischémique : à partir de la cohorte LyTONEPAL

Abstract : Background: Some studies suggest differences in the type of lesions observed on Magnetic Resonance Imaging (MRI) in term and preterm neonates with Hypoxic-Ischemic Encepahlopathy (HIE). Identifying the impact of gestational age (GA) on the type of lesions would allow clinicians to better understand and treat this pathology in this population. Objectives: The primary objective was to determine whether GA has an isolated impact on the occurrence and type of cerebral lesions detected on MRI in the first 12 days of life. Methods : An analysis of prospectively collected data was performed in newborns between 34 and 42 weeks of GA with HIE from a French cohort identifying HIE cases between 2015 and 2017. The presence and location of brain injuries observed on MRI were compared between five GA subgroups. Results: In this study, 626 infants were included, including 94 preterm infants born before 37 weeks of GA. White matter (WM) lesions were the main lesions in preterm infants, especially between 34 and 35 weeks of GA (51%, p < 0.01). In the subgroup of infants with WM injuries, the association between GA and the occurrence of lesions persisted even after adjustment for sex, therapeutic hypothermia (TH) and clinical severity of HIE (OR 2.27, 95% CI [1.11-4.65], in the subgroup of 34-35 weeks’ GA). Conclusion: GA seems to have an isolated impact on the increased frequency of WM lesions in the late preterm infants. Further randomized studies are needed to assess the benefits of TH in this population.
