Les effets de l'exercice aquatique (balnéothérapie) sur la fatigue chez les patients atteints de sclérose en plaques

Abstract : Introduction: Multiple sclerosis is a chronic and incurable disease of the central nervous system, affecting 2.8 million people worldwide. Present in ¾ of patients, fatigue is one of the most disabling symptoms of this disease. The management of fatigue is based on advice and different protocols, including aquatic exercise. Objective: The aim of this review is to evaluate the effects of aquatic exercise, balneotherapy, on fatigue in patients with multiple sclerosis, in order to provide an evidence-based practice. Method: Searches were conducted using 3 scientific databases (PubMed, Pedro, Cochrane) via a search equation. The article selection process was established beforehand, and resulted in the inclusion of 4 studies (RCTs) whose methodological quality was assessed by the Pedro scale. Results/Discussion: Effect sizes were large, with a mean intergroup difference for MFIS of -22.05 and -10.01 for FSS. Except for the Kooshiar et al. 2015 study, the confidence intervals are significant and clinically relevant as the absolute values of the bounds are greater than the corresponding MCID. The average PEDro score across all studies is 6.25/10 which is reasonable despite the presence of potential bias. Conclusion: This review shows a significant reduction in fatigue and its impact by aquatic exercise compared to a control group that did not undergo any intervention with the MFIS. With regard to the FSS, the meta-analysis does not show that this result is extrapolable to the target population but only that this reduction in fatigue and its impact is significant for the samples of both studies. Therefore, further reviews should be carried out on the same topic, while others should compare the effects of aquatic exercise on fatigue with other therapies used.
