Incidence des infections sur fractures ouvertes des os longs au CHU de Caen de 2016 à 2018 : étude épidémiologique, facteurs de risque et analyse de pratique

Abstract : Open fractures are one of the major challenges of trauma surgery. The main complication is infection. The aim of our study was to observe the incidence of infections in open fractures, risk factors for infection, epidemiology of open fractures and bacterial epidemiology of infections. 179 patients with at least one open long bone fracture treated at the Caen University Hospital were retrospectively included between January 2016 and December 2018. The primary endpoint of the study was the incidence of infections. The study of risk factors for infections was subjected to Fisher's exact tests and Student's t tests. The infection rate was 22.6% (2.2 to 53.8% depending on Gustilo's stage). The major risk factor for infection was the Gustilo classification of the fracture. The epidemiological distribution of patients was bimodal with maximum incidences for young men (20-30 years) and elderly women (80-90 years). AVP represented the first fracture circumstance (53%). The tibia represented the first farcture site (44%). The two main germs found were Staphylococcus aureus (42%) and Enterobacter cloacae complex (40%). The initial antibiotic prophylaxis was overwhelmingly amoxicillin and A.clavulanic acid (94%). The open fracture infection has a high incidence in CHU of Caen. Improvements are possible for the management of these fractures by acting on the type and duration of the surgical procedure, as well as on the modalities and duration of the antibiotic prophylaxis.
