Évaluation des connaissances et des pratiques cliniques concernant la prise en charge des prolapsus génitaux et de l'incontinence urinaire de la femme en médecine générale sur le territoire de l'ex Basse-Normandie

Abstract : Introduction: pelvic-perineal disorders are very common and bring together different pathologies including urinary incontinence (UI) and pelvic organ prolapse (POP). About 1 in 9 women will have surgery before age 80 for a POP and 15 to 50% of women have UI according to studies. These disorders impact the quality of life on the psychic, social and sexual levels. They are associated with taboos limiting consultations for these issues. Many general practitioners (GPs) are not comfortable with these pathologies. The aim of the study was to assess knowledge and clinical practices concerning the management of POPs and UI in women, in general medical practice, in the territory of the former Basse-Normandie. Method: quantitative cross-sectional descriptive study by questionnaire. The population was formed by drawing lots for MGs; then the questionnaire was distributed to the masters of university internships in the territory by email, web or by telephone. Results: among the 503 GPs solicited, 81 responses were usable (16.1%). 97.5% of GPs offered at least partial gynecological follow-up. 95.1% knew how to recognize a POP and 85.2% prescribed perineal rehabilitation in 1st intention. Only 1/3 had ever prescribed or handled a pessary and, 1/3 didn't know about them. 100% recognized exertional UI and 81.5% recognized Urge UI. 70.3% of GPs mentioned obstacles to their care (taboo and lack of training (72.8%)). Conclusion: GPs feel concerned by these pathologies but express difficulties in taking charge due to taboos and lack of training.
