OPERA 2 : On The Phone Respiratory Rate 2 : étude pilote de faisabilité d’une mesure rapide de la fréquence ventilatoire en régulation médicale à l’aide de l’application téléphonique R-Rate chez des patients appelant le centre 15 de l’Isère pour dyspnée

Abstract : Introduction. Dyspnea is a very frequent subjective symptom and presents complex physiopathological mechanisms. It could be results frommany respiratory diseases, but also results from many other chronicles diseases. Respiratory rate is a reliable and simple clinical sign with a strong correlation with patient’s illness severity and mortality.Many mobile phone apps can take a respiratory rate measurement. The most reliable is R-Rate app. The aim of this pilot study is to assess the feasability of quick respiratory rate measurement in patients calling french medical regulation for dyspnea in Isère county. Material and methods. OPERA 2 is a pilot, observational, prospective and single center study. The recruitment begins in july, 2021, the 28th and the study is still in progress yet. Patients were recruit only in the CHU Grenoble-Alpes. They should to call French medical regulation to dyspnea, or the doctor need to identify the dyspnea during the medical speech. To get a R-Rate value during the call, another people is listening the speech and make a single attempt to obtain respiratory rate value. 70% success was expected. Results. With 41 patients included during the recruitment, we only got 56% success of R-Rate measurement. The R-Rate median was around 25 cycles per minute. 82.6% (19/23 patients) have respiratory rate strictly upper than 20 cycles per minute but only 17.5% (4/23 patients) are considered as critical patients. Several limiting factors are highlighted : the too short listening of respiratory cycles and difficulty hearing. Conclusion. R-Rate app can’t be actually use as single way in order too rank critical patients with respiratory rate in French medical regulation
