Trois clusters de variables prédisent l’efficacité d’un programme de réentrainement à l’effort sur le retour au travail à 3 mois dans une population de lombalgiques chroniques

Abstract : Objective: Chronic low-back pain (LBP) is a major health and socio-economic problem. Functional restoration programs (FRP) have been developed to promote socio-professional reintegration of patients with important work absenteeism. The aim of this study was to determine effectiveness of FRP in a group of 183 chronic low back pain (LBP) patients and to determine the predictive factors of return to work. Methods: This monocentric retrospective cohort study included 183 patients with chronic LBP and work disability. Pain, quality of life, functional disability, mental health and physical condition were evaluated at baseline, at the end of the program and at 3 months follow-up. Professional status, duration of sick leave, work accident or occupational disease declared, working conditions and work satisfaction were collected at baseline. Primary objective was return to work. Results: 76% of the patients returned to work at 3 months follow-up compared with 36% of patients at work at baseline. Pain, quality of life, functional disability, mental health, and physical condition were all significantly improved a 3 months follow-up. Shorter sick leave, being at work at baseline, a better functional status and a better mental health at baseline predict return to work at 3 months. Conclusion: This FRP shows positive results in terms of return to work for chronic LBP patients with prolonged work absenteeism. Considering predictive factors of success, patients should be refered to such programs at an earlier stage of the disease.
