Prescription d’antidépresseurs pendant la grossesse : une revue des bénéfices et risques basée sur les méta-analyses, et caractérisation de l'hypotonie néonatale

Abstract : Anti-depressants prescription prevalence during pregnancy is steadily increasing, deemed 2.5% in France and more than 5% in the United States. Knowing the potential adverse effects of anti-depressants is a public health issue, for pregnant women, foetuses, newborns and developing children. The first part of this thesis presents a review of the literature based on meta-analyses about the risks and benefits of anti-depressants prescription during pregnancy. The meta-analyses include the results of several studies having the same objectives, which reinforces the statistical impact and the precision of a result. This is therefore a reference method to assess the safety of the prescription of medicines. This meta-review mainly identifies that: 1) the benefits for pregnant women are evident in case of a serious or recurring depression, 2) the most frequent risks affect the newborn and are minor, 3) when the biases are controlled, the risks attributed to antidepressants become non-significant. These results also suggest that the benefit/risk balance is all the better as the recommendations on indications and on the choice of molecules are respected. The second part of this thesis presents our contribution to a retrospective study describing the risks of neonatal hypotonia following in utero exposure to anti-depressants. This study is based on two pharmacovigilance databases: Vigibase® (the WHO database), and the BNPV (Base Nationale de PharmacoVigilance française – The National database for French pharmacovigilance).
