Les critères diagnostiques d’addiction dans l’usage problématique du sexe

Abstract : In scientific and non-scientific literature, different names are used to refer to excessive sexuality. The development of internet support over the past decades has considerably increased the availability and accessibility of pornographic sites, images and videos with the associated risk of promoting the development of sexual addiction. Nevertheless, the existence of addictive behaviors with regard to sexual activity is today the subject of debate and controversy and the question of the similarities between problematic and compulsive use of sex and other addictions arises. A better knowledge and description of this disorder appears necessary to improve identification and management. The objective of this work was to examine the current state of knowledge on the diagnostic similarities between problematic sexual behavior and other addictions, according to the Goodman criteria and the DSM-5 criteria. We carried out a systematic review of the literature aimed at evaluating the diagnostic criteria for addiction in problematic sex use by following the PRISMA method. The studies were selected from the online bibliographic databases PUBMED, PSYCINFO and EMBASE without limitation of publication date. A total of 15 studies were included in the review. Most studies found similarities between diagnostic criteria for substance addiction and problematic sex use. The most frequently identified criteria associated with problematic sex use were craving, loss of control, and continued use despite negative consequences. Experimental studies showed that exposure to sexual stimuli generated episodes of craving in problem users compared to occasional users. The methodological quality was limited by the heterogeneity of the assessment tools and the lack of representativeness of the samples. Additional studies are still needed to better identify the addictive dimension of the behavior and the association with other addictive behaviors including chemsex.
