État des lieux local de l’accès à une consultation d'onco fertilité chez les femmes jeunes traitées pour un cancer du sein

Abstract : Objectives: According to the bioethics law of 2004, a fertility preservation consultation must be systematically proposed to all patients of childbearing age before they are exposed to a potentially gonadotoxic treatment. The main objective of this retrospective study is to evaluate the proportion of patients aged 18 to 40 years treated for breast cancer who have received a fertility preservation consultation in Grenoble. This study is a prerequisite for the setting up of a "fertility preservation" Multidisciplinary Consultation Meeting (RCP) at the Grenoble-Alpes University Hospital (CHUGA). The secondary objective of this study is to evaluate the frequency of recourse to a fertility preservation technique following this consultation. Method: This is a retrospective descriptive multicenter study not involving the human being. The initial list of patients was obtained from the list of patients registered in the RCP of oncology for breast cancer at the Grenoble University Hospital or at the Daniel Hollard Institute. The information was collected from the patients' paper and/or computerized files. The data were extracted from consultation reports, letters from oncologists or surgeons in the different structures of the Grenoble area, as well as from the reports of the two oncology RCPs. Results: From 2015 to 2020, of the 139 patients at GHM and 88 patients at CHUGA under 40 years of age newly diagnosed with invasive breast cancer, 48 patients (34%) at GHM and 29 patients (32%) at CHUGA were seen for fertility preservation consultation. Of these 77 patients who had access to an oncofertility consultation, 20 patients (25.9%) had recourse to a fertility preservation technique. The rate of referral to a consultation varied according to the age of the patients (100% of patients aged 18 to 24 years were referred compared with 15.92% of patients aged 35 to 40 years, p<0,001) and the use of a consultation depended on parity (p<0,001). Conclusion: This study provided local epidemiological data on the proportion of women referred for fertility preservation counseling. It demonstrates a significant disparity in access to this consultation. There are still many actions to be undertaken in the region in terms of informing the various parties involved in the oncological management of patients, so that the clinical-biological MPA center that carries out the oncofertility and fertility preservation consultation activity can best fulfill this mission.
