Apport de l'IRM thoracique dans la prise en charge de la fibrose pulmonaire idiopathique

Abstract : CT scan is a key exam in idiopathic pulmonary fibrosis diagnosis defining the UIP pattern according to the last international guidelines published in 2018. However, it is a radiating exam which solely provide morphological information. Lung MRI is a developing technic which could be non-inferior to CT scan to describe UIP patterns and which would be able to provide functional data. We conducted a monocentric prospective study at CHU Bordeaux from 2016 to 2020 including 60 subjects (18 UIP, 7 possible UIP, 15 stage IV sarcoidosis and 20 patients with disease without reticulations or healthy subjects). Each patient performed a CT scan with millimetric slices, and a lung MRI with UTE VIBE sequence before and 15 minutes after gadolinium injection 1 BLADE sequence. Diagnostic performances of lung MRI for each of the 4 diagnostics criteria forming UIP pattern such as sub pleural and basal localization of lesions, reticulations, honeycombing and absence of excluding lesion had sensitivity and specificity better than 65% and 75%, respectively. Lung MRI was non inferior to CT scan to describe UIP pattern lesions and provided functional information which would be interesting in the future to sort different types of patients and to evaluate treatment answers.
