Efficacité de la formation des professionnels de santé en hypnose thérapeutique sur la réduction de la douleur lors de la pose d’un dispositif intra utérin chez la patiente nullipare : étude prospective observationnelle comparative

Abstract : Contraception is a major public health issue. Since the pill crisis in the 2012s, there was a great increase in the use of intrauterine devices. There are still many obstacles to the prescription of this contraception, the main one being the fear of pain during the insertion. This fear is predominant in nulliparous patients, who are most at risk of pain during insertion. There are currently few effective premedications for reducing the said pain. Patient information and pain management using non-pharmacological methods such as therapeutic hypnosis, would have a positive impact on pain during placement. The main objective of our study is to describe the effectiveness of therapeutic hypnosis training of healthcare professionals in reducing pain during the insertion of the device. Method: This is a prospective observational study, comparing two groups of nulliparous women consulting for an intrauterine device placement. Two groups of recruiting health professionals were formed according to their training in therapeutic hypnosis, or not. The inclusion criteria are: any nulliparous patient, adult, not opposing the participation in the study, speaking French and having chosen a contraceptive by intrauterine device with copper or hormonal, including emergency contraception by the copper device.The primary endpoint is the evaluation of the patient’s pain during insertion of the intrauterine device, using a numerical scale from 0 to 10. A clinically significant difference in pain is considered if it is greater than 1.3 point. Results: 126 patients were included, 49 in the hypnosis group and 77 in the without hypnosis group. The resented pain during insertion of the intrauterine device is less important when the insertion is performed by a health professional trained in therapeutic hypnosis (delta EN 1.5 points out of 10, p <0.001). The difference is clinically significant. Conclusion: Hypnosis is a good therapeutic et can allow better pain management when placing an intrauterine device. Promoting the use of this reliable and effective contraceptive method is very important, especially in nulliparous patients, because it is still being underused.
