Vécu et représentations de la maladie chez les diabétiques en grande précarité

Abstract : Introduction - The prevalence of diabetes is highest and its management seems sub-optimal for people in precarious social situations. Very few studies give voice to diabetic people living in extremely precarious situations. The aim of this study is to analyze experiences and representations that diabetic people in extremely precarious situations have of their disease, in order to formulate ways to improve the medical follow-up of these people and the control of their disease.
Methods - A qualitative study was carried out in Île de France, using semi-structured interviews among diabetic people in very precarious situations, attending hospital emergency departments, medical consultation for destitute patients and “post-hospital medical respite care” (“Lits Haltes Soins Santé) for homeless people.
This survey was conducted from November 2018 to April 2019. A thematic analysis of these interviews was conducted.
Results - Nine interviews were conducted. The difficulties identified are living conditions, seclusion, financial difficulties and priorities for daily survival, but also the feeling of having been discriminated against by some doctors, the complexity of administrative procedures, the poor understanding of the French healthcare system and the lack of social health insurance.
Conclusion - To improve the management of diabetes among destitute population, efficient collaboration between healthcare and social workers, promotion of patient education, adaptation of good practice guidelines and awareness of primary healthcare professionals, especially general practitioners, seem essential.
