Issues maternelles et néonatales des déclenchements à la suite d'une demande maternelle comparées à celles des déclenchements pour autre indication après 41 SA : étude rétrospective unicentrique sur un an au CHU de Bordeaux

Abstract : Objective: compare delivery mode as well as maternal and neonatal morbidity between labor induction at 41 weeks of gestation for maternal request or labor induction at 41 weeks of gestation for other indication. Material and methods: unicentric retrospective observational study related to 448 patients who went through labor induction after 41 weeks of gestation. The primary outcome was the rate of caesarean delivery. Secondary outcomes were maternal and neonatal morbidity. Results: a total of 448 patients were included, of whom 420 were in the “other indication labor induction” group and 28 in the “maternally requested labor induction” group. The proportion of caesarean delivery was similar in the “maternally requested labor induction” and “other indication labor induction” groups after 41 weeks of gestation (21.4% vs 28.1%; p=0.52). After adjusting for confounding factors, induction by maternal request was not associated with a reduction in the caesarean section rate (ORa=0.8, CI95% 0.3-2.3), nor with a reduction in maternal morbidity (ORa=0.6, CI95% 0.3-1.4) or neonatal morbidity (ORa=1.1, CI95% 0.1-8.6). Conclusion: labor induction for maternal request after 41 weeks of gestation is not significantly associated with a reduction of the rate of caesarean compared to labor induction for other indication after 41 weeks of gestation. Maternal and neonatal morbidity is comparable between labor induction for maternal request and labor induction for other indication after 41 weeks of gestation.
