État des lieux de la prise en charge entre la ville et l'hôpital du patient adulte atteint d'un syndrome drépanocytaire majeur à la Martinique : une étude qualitative exploratoire

Abstract : INTRODUCTION: The main objective was to know the actions, the expectations, the difficulties of the current care, from the point of view of hospital doctors, general practitioners, and patients. METHODS: A qualitative study was carried out. It consisted of seven semi-structured interviews with general practitioners, eight with hospital practitioners and three focus groups with sickle cell patients. RESULTS: The central role of the Integrated Center for Sickle Cell Disease (ICD) was well recognized despite the recognition of a lack of organization. The management of emergencies was not considered satisfactory. General practitioners expressed difficulty in contacting the ICD for medical advice. There was a lack of training noted by all the doctors interviewed. It emerged that there was a need to improve the care pathways for sickle cell patients. The opinions regarding the creation of a health network were divergent and raised questions as to how to improve this course. Primary care and hospital care cooperation was claimed by general practitioners but considered with caution by hospital doctors. DISCUSSION: We looked for the form that the organization of the sickle cell patient care path could take. Two approaches emerge : promoting a health network or optimizing clinical pathways. CONCLUSION: The creation of a network of primary and secondary care services is not necessarily the only solution in the current state of the Martinican health system. This would require the consultation of the main stakeholders to be consistent with the regional health project.
