Déclenchement de citoyens sauveteurs via une application mobile par le SAMU de Bayonne lors d’un arrêt cardiaque

Abstract : Background: out-of-hospital cardiac arrest (OHCA) is a major health issue in France. Mortality remains very high. Reducing time to cardiopulmonary resuscitation (CPR) and defibrillation by citizens through smartphone applications improves survival rate. Objective: the main objective was to determine the rate of SAUV Life’s activation when OHCA occurred in the area of Bayonne’s dispatch center. Secondary objective was to study OHCA epidemiology in this zone. Methods: this monocentric, observational and retrospective study was conducted during 2 years (from 05/18/2018 to 05/17/2020). All OHCA treated by Bayonne’s dispatch center were included. 2 populations were compared: one where SAUV Life was activated and another where SAUV Life wasn’t activated. Results: 336 OHCA were included: 304 didn’t have SAUV Life activated, 32 had SAUV Life activated. SAUV Life’s activation rate was 9,5%. Median age was 71 years, OHCA were mainly men (69%), occurred at home (79%), had a medical cause (87%), had a non shockable initial rhythm (81%). Return of spontaneous circulation was obtained in 22% of the cases, survival rate to hospital was 20%, survival rate to hospital discharge was 5%. OHCA were witnessed by a bystander in 49% of the cases. Bystanders applied an automated external defibrillator (AED) in 3% of the cases. We found significantly more men, OHCA occurring at home, cases where CPR was performed before the call to the dispatch center, intravenous adrenaline use and intubations in the group where SAUV Life was activated. Conclusions: SAUV Life’s activation rate by Bayonne’s dispatch center is lower than in other studies. Creating an activation protocol for this application seems mandatory. OHCA characteristics in the area of Bayonne’s dispatch center are similar to other studies.
