Freins et leviers à la prise en charge en soins premiers des psychotraumatismes des exilés en Île-de-France

Abstract : The mental health of exile population are often sidestepped in primary care. The objective of our study is to determine the factors related to general practitioners which may have an impact on primary care of mental health of exile patients affected by psychotraumatic disorders. A qualitative study was conducted with 20 physicians practicing in Île-de-France by semi-directed interviews between July and December 2018. Several factors interfering in the primary care of psychotrauma of exile patients were identified from 12 interviews with general practitioners working in distinct structures. Their life experience and the organization of the healthcare system hindered their access to medical care. The intricacies related to their status as migrant and psycho-socio-cultural background made the consultation of this population time-consuming, especially when language and cultural barriers hampered the communication, particularly at a psychological level. Furthermore, personal and professional involvement modulated general practitioners’ sensitivity and apprehension towards psychotraumatic disorders of exiles. On the other hand, patients’ lack of trust in the physician and obstacles in the referral to specialists prevented an optimal management of mental health for this vulnerable public in primary care. Primary care practitioners experience numerous difficulties in the care of psychotraumatic disorders of exile patients on the structural, socio-cultural and emotional dimensions. Measures acting on clinical training, practice, and organization are needed to improve the primary care of mental health of this population.
