Le toucher vaginal lors de l’examen de sortie de la maternité : enquête d’opinion auprès des sages-femmes françaises

Abstract : Introduction: In France, the vaginal examination (PV) before discharge from maternity ward is practiced out of any recommendations, in the context of a global decrease in the use of this examination procedure during pregnancy. Background: The aim of this study was to better evaluate the place of this practice in the postpartum period, as well as the motivation of midwives to perform it and their possible request for a CPR. Methods: This opinion survey is a descriptive study with an evaluative purpose, carried out through the flash distribution via Facebook® of a questionnaire developed with the REDCAP® software. Results: Out of 456 usable responses, the results showed that 23% of midwives perform this procedure systematically (including 19% on sutured perineum), in 99% of cases in search of forgotten compresses. 76.30% would be in favor of new recommendations (13.80% would like to see PV generalized in the postpartum period, while 62.70% would like to see it reduced). It seems that midwives base their practice more on their experience than on scientific data. Conclusion: The predominantly extrinsic motivations of midwives in favour of PV before discharge of maternity ward are based on the detection of potential human errors, and they point out failures and the need to pay attention to them. It was proposed to add to the forensic partogram the checkbox "vaginal touch: done / to be checked" and to promote the sterile mouse for sutures. A more precise and representative study of midwives' motivations and patients' feelings should precede the publication of new recommendations on the subject.
