Quelle est la prévalence de la carence en vitamine B12 chez les patients végétaliens ? Une revue de la littérature

Abstract : Introduction: There exists a fair risk of vitamin B12 deficiency among vegan patients. This particular diet stems from a concern for well-being as well as a quest for more ethical means of sustenance. The notion of vitamin B12 deficiency can sometimes be a source of anxiety for these patients. The absence of consensus make it so that caring for those patients can be rather tricky. The aim of our study is to assess the prevalence of vitamin B12 deficiency in vegan patients via a systematic review of literature. The secondary objective is to assess the need for regular biological monitoring in the absence of deficiency symptoms. Process: Systematic literature review using the Cochrane Handbook method on the MEDLINE, LiSSa, DUMAS, SUDOC and Google scholar databases over the year 2020. Two independent readers. Results: 487 references were found from the different databases, with a final inclusion of 17 articles for data collection. The prevalence of vitamin B12 deficiency is found to be highly variable, ranging from 0 to 78% in healthy vegan adults devoid of symptoms. Regular vitamin B12 supplementation seems to reduce, yet not exclude, the risk of deficiency. Discussion: Few articles dealing with the subject and showcasing great heterogeneity of methodology. Small population size, with sometimes incomplete data collection on population characteristics, exact diet. Conclusion: There is currently a need for consensus on the monitoring of these patients, which population is growing. More studies are to be hoped for. It seems sound to offer regular biological monitoring.
