Évaluation des pratiques en médecine générale concernant le dosage et la supplémentation en vitamine D des patients âgés

Abstract : The interest for vitamin D is growing, and the number of publications on this subject has greatly increased. The number of dosages of vitamin D is also rising sharply, motivating a revision of its indications. Dosage indications in the elderly are not always clear.
Methods: in the context of an observational retrospective descriptive analytic study, we studied 150 hospital patient files from January to December 2018 in the Perioperative Geriatrics Unit of Pitié-Salpêtrière Hospital. All antecedents and prescriptions from the patient's referring physicians were collected, the Rockwood, CFS and CIRS scores were calculated for each of them. A biological assessment with the 25,0H-vitamin D assay and albuminemia was taken at the entrance. The Chi2, Fischer and Student tests were used to investigate relationships between patient characteristics and vitamin D levels. The referring physicians were contacted by telephone to determine their attitude to the dosage and vitamin D supplementation. We examined whether patients' vitamin D levels were associated with asking for a pre-hospital dose.
Results: we did not demonstrate a significant statistical relationship between vitamin D levels and certain patient characteristics. The indications used by the treating physicians for the dosage in their elderly patients were: age, multiple comorbidities, history of fractures and osteoporosis, sedentary lifestyle, recurrent falls, light phototype, undernutrition, menopause and chronic pain. The vitamin D level was significantly higher at the entrance when the doctors asked for an assay in their elderly patients.
Conclusion: larger studies would be interesting to study the impact of vitamin D dosing, particularly on the prescribing modalities of supplementation, to optimize average vitamin D levels in the general population.
