Évaluation de la prise en charge médicale et de la connaissance des patients de la conduite à tenir en prévention secondaire d’une anaphylaxie

Abstract : Objective: to evaluate the medical management and the knowledge of the patients of the behavior to take in secondary prevention of anaphylaxis.
Materials and methods: we conducted a pilot, descriptive, multicentre, prospective, non-interventional, questionnaire-based study at the Porte de Vanves MSP and in the emergency department of the Saint-Joseph Hospital, located in the 14th district of Paris, over a period patients included were over the age of 18, having previously experienced an anaphylaxis reaction excluding patients presenting for an allergic or anaphylactic reaction or presenting an unstable condition. For any patient consulting the antecedent of anaphylaxis was sought if it was present a questionnaire was given to the patient so that he could fill it himself after the consultation. The main criterion of judgment was the medical management of the patients after a first episode of anaphylaxis according to the recommendations of the HAS based on the prescription of an adrenaline auto-injector, the distribution of A tip sheet of allergen eviction an allergology consultation.
Results: 60 patients were included: 43 patients in the emergency department and 17 patients in the MSP. 75% of them presented a drug-induced anaphylactic reaction, 30% food, 11.7% with a bite of hymenopter and 3.3% with an unknown allergen. The results show 16.7% of prescription of adrenaline injector; 15% of distribution of a card advice and 45% of consultation of allergology.
Conclusion: this study shows a weak application of the recommendations for the secondary prevention of anaphylaxis.
