Dysfonctions sexuelles chez les patients atteints de schizophrénie

Abstract : Sexual dysfunctions affect 34 to 67% of patients with schizophrenia, alter both patients and their partners quality of life, and modify the course of the disease. Purpose of this thesis was to conduct a non-exhaustive review of the literature using the Pubmed and Cochrane databases. A total of 91 articles published until 2020 were included in this review. The main root cause of these disorders is iatrogenic due to the anti-dopaminergic action of antipsychotics, leading to reduction in motivation and hyperprolactinemia. Other targets of antipsychotics are also associated with impairment of sexual function, such as serotonergic, adrenergic, histamine and cholinergic receptors. These sexual dysfunctions are also present before the start of treatment in patients presenting their first psychotic episode, or in patients at high risk of developing them, which is in favor of a physio-pathological origin of these disorders. The psychiatric disorder plays a role in sexual function through negative psychotic symptoms, institutionalization and self-stigmatization. The influence of comorbid pathologies is also studied, including cardiovascular, addictive and psychiatric pathologies. An algorithm for the management of these disorders has been suggested in some studies, such as reducing the dosage of the treatment, switching to an antipsychotic that is less likely to cause this type of side effect, or adding Aripiprazole. Adjuvant treatments are also possible, such as hormonal treatments or phosphodiesterase 5 inhibitors. Finally, psychotherapeutic treatment would seem to be beneficial.
