Caractérisation morphologique, immunohistochimique et moléculaire des tumeurs mésenchymateuses avec réarrangement de NTRK

Abstract : Introduction: NTRK-fused tumors have drawn attention these last years because of the development of a targeted therapy but these tumors have a wide unspecific morphological spectrum making their diagnosis difficult. Our study described the morphological and molecular characteristics of 36 tumors with NTRK fusion to further document this entity. Material and method: 122 fibroblastic/myofibroblastic tumors whose initial diagnosis belonged to the NTRK fused tumors spectrum, to their differential diagnosis or remained unclassified were removed from the archives of the Institut Bergonié or the Bordeaux University Hospital. A histological review was performed for each case with additional immunohistochemistry and/or molecular biology analyses depending on the material availability. Results: NTRK rearrangements were found in six main morphological patterns: unspecific spindle cell tumors with fascicles of variable architecture and a collagenous stroma, infantile and adult fibrosarcomas, hybrid tumors combining several histological patterns, dermatofibrosarcoma protuberans, inflammatory myofibroblastic tumors and a myofibroma. Almost all these tumors had a co-expression of S100 and CD34 on immunohistochemistry. The pan-TRK antibodies study reported a sensitivity of 82,9% for pan-TRK (Abcam) and 80.6% for pan-TRK (Cell Signaling). Most of the fusions involved NTRK1 and less frequently NTRK3. Many fusion partners were reported. Conclusion: co-expression of CD34 and S100 in a spindle cell tumor with unspecific or hybrid aspects is suggestive of NTRK rearrangement. Pan-TRK antibodies have an incomplete sensitivity for the detection of NTRK fused tumors.
