Parcours thérapeutique et facteurs de risque de complications postopératoires chez les prématurés de moins de 29 semaines d'aménorrhées opérés d'un canal artériel persistant

Abstract : Background and purpose: The ductus arteriosus is a shunt between the pulmonary artery and the aorta that closes within 3 days of birth. It becomes patent in neonates with a very low birth weight, with respiratory and digestive complications. Ibuprofen is the most widely used treatment and surgery is suggested as the last resort. However, there are no official guidelines. The main objective of our study was to describe the characteristics of the patients who underwent surgery and to determine the risk factors of postop complications. Methods: The study was conducted retrospectively and included all premature infants under 29 weeks of amenorrhea treated in neonatal intensive care unit who underwent surgery of a patent ductus arteriosus between 2010 and 2019 at the Caen University Hospital. We split the patients into 2 groups, "Complications" and "No complications". Results: 62 neonates were included. 31 patients had at least 1 postop complication. The ultrasound diagnosis was made earlier in the "Complications" group. Ibuprofen is used less and less over time (p=0.004). The occurrence of complications remains unchanged over the years (p=0.964). Infectious complications represent 2/3 of the total. Only 2 patients had post-ligation cardiac syndrome. Six patients died, ie 10% of the population, and more in the "Complications" group (6 vs. 0; p=0.012). Conclusion: Management of patent ductus arteriosus in premature infants is progressing with decreased use of Ibuprofen and surgery. No risk factor could be determined.
