Incidence de l'ostéonécrose des maxillaires de patients sous XGEVA® (Denosumab 120mg) : étude de cohorte rétrospective

Abstract : Background: Osteonecrosis of the jaw ONJ is described as an expected adverse effect of bisphosphonate and denosumab. However, the incidence rate of ONJ of denosumab 120mg is not clearly defined, especially after close oral check-up. The aim of this retrospective study is to estimate the crude incidence rate of ONJ related to dénosumab 120mg in patients, who undergo dental preventive measure. Methods: We included legal age patients who had an oncologic indication for denosumab 120 mg, and an oral check-up (pre-treatment and after initiation) at the Dental Care Centre in Rennes University Hospital. The primary endpoint was the ONJ. Estimation of the crude incidence was performed per 100 patient-year. Results: 251 were included in the study, 183 women and 68 men, with an average exposure duration of 9 months. Ten ONJ were observed (stage 0 according to AAOMS for 4 patients and stage 1 for the 6 others), with an mediane exposure time of 12 months (Q1: 11,25 - Q3: 18). The incidence rate per patient year was 5.1 per 100 person-year (IC 95% : 1,9-8,3). Conclusion: Our study shows the importance of implementing preventive oral measures combined with regular oral check-up for patients treated by denosumab 120mg. Oral close monitoring is necessary for ONJ prevention, early detection and care.
