Prise en charge par les médecins généralistes bretons pratiquant l'échographie, des suspicions de thrombose veineuse profonde des membres inférieurs

Abstract : Introduction: French general practitioners are showing increasing interest in a tool that is not currently used in practice: ultrasound. The purpose of this study was to assess in breton general practitioners who perform ultrasound, the confidence they place on venous doppler of the lower limbs, if they do so in the event of a suspicion of deep vein thrombosis. Method: An online survey was sent through the 4 breton departmental councils of the french medical order, as well as to graduates of the DU in ultrasound from the Faculty of Brest. The inclusion criteria were to be a Breton general practitioner and to perform ultrasound regularly, but not exclusively. Results: 52 complete responses were collected, 40 could be included. 21 physicians usually perform Doppler ultrasound of the lower limbs but then refer most of the time to the angiologist, 14 perform it without any control requested from the specialist and finally, 5 do not perform this type of ultrasound. The Khi 2 test shows a statistically significant difference (p = 0.04); Moreover, no clear correlation was found between possible recourse to a specialist and the socio-demographic data as well as the initial training in ultrasound of the participants. The use of additional examinations does not seem to change in any of the three groups either. Conclusion: This important recourse to the specialist is a reflection of the lack of confidence of the general practitioners in their ultrasounds. Training and practice should solve this in the future, we can only encourage general practitioners to persevere to better train, for which Brittany could become a privileged training center in France.
