Évaluation de l’énergie optimale en tomodensitométrie double énergie pour la détection des chambres de résorption osseuse au pourtour du matériel d’ostéosynthèse rachidien

Abstract : Purpose: To evaluate the optimal energy of spectral Computed Tomography for metal artifact reduction in patients with pedicle screw loosening. Methods: 46 patients with pedicle screws loosening underwent spectral CT examination. Virtual monochromatic images used for diagnosis (70-80 keV) and five sets of virtual monochromatic images at high energy (100–140 keV) were obtained. The standard deviation (SD) of CT number in pedicle screw loosening and around a screw without loosening was measured to generate artifact index (AI) and Beam Hardenning Rate (BAR). The same measure was performed in worst level degraded by metal artefacts. Objective assessment including AI, BAR, CT number and SD value was performed with ANOVA test and a linear regression model for continuous dependent variable with adjustment on hyperdense and hypodense AI. Results: The AI of screw loosening regions decreased as keV increased such as for the worst artefact. In ANOVA test, the 120 keV images provided an accurate CT number for analysis of pedicle screw. But in linear regression with adjustment on the most pronounced artefact,110 keV seems to be the real accurate energy in virtual monochromatic images.Conclusion: Virtual monochromatic images at 110 and 120 keV provided an accurate CT number for analysis of pedicle screw loosening in Dual Energy Computed Tomography with Spectral technology.
