Mise sous pompe à insuline des patients diabétiques en hospitalisation de jour : évaluation clinique et médico-économique comparative avec l'hospitalisation de semaine

Abstract : Introduction: Diabetes is a frequent chronic pathology defined by hyperglycemia. Different forms exist, including type 1 diabetes, for which treatment with insulin therapy is always necessary. Type 2 diabetes, for which the treatment initially includes oral antidiabetics and can then be intensified by insulin therapy with subcutaneous injections of insulin. When this regimen is no longer sufficient for diabetes control, a pump can complete the treatment. Material and method: Pumps are usually placed in weekday hospitalization (HDS). At Caen University Hospital, another method of initiation, in day hospitalization (HDJ), has been tested since 2015. Our study compares these two methods and aims to demonstrate the non-inferiority of HDJ management. This study is retrospective monocentric and compares two groups of patients put on pump between 2015 and 2020, associated with a medico-economic analysis of the costs related to these different hospitalizations. Results: Our analysis compares 47 patients in HDS and 49 in HDJ and finds a comparable delta of HbA1c, before and after pump placement, as well as an equivalent number of hypoglycemias, ketoses, or adverse events in our two groups. The medico-economic analysis found a major difference in costs, which were lower in HDJ than in HDS. Conclusion: HDJ is therefore as effective as HDS for insulin pump placement and reduces the healthcare costs associated with diabetes management.
