Intérêt de la transfusion pré-hospitalière dans la prise en charge des hémorragies post traumatiques : étude du protocole de transfusion du SAMU 64A

Abstract : Background: severe trauma is the leading cause of death in the 15-44 years population. Hemorrhage is the first preventable death cause after severe trauma. The evidence of pre-hospital blood transfusion to treat post traumatic hemorrhage is still under discussion. Objective: first to establish the rate of transfusion when the pre-hospital transfusion protocol is activated by the SAMU 64A then to study the interventions for traumatic causes. Methods: monocentric, observational and retrospective study was conducted from June 2020 to December 2021. Including all primary interventions for wich ones prehospital transfusion protocol was activated. Physiological datas, medical management and duration of interventions were collected and analyzed. Results: data from a total of 109 interventions were analyzed : 25 interventions lead to a pre-hospital transfusion (22,9%). Median age was 40 years and composed of 76,1% men. Trauma was the main cause of pre-hospital transfusion protocol activation (80,7%).Blunt traumas were the most frequent (60,2%). Shock index ≥ 0,8 was found for 73,6% of transfused patients. Time on scene seemed to be longer when patients with traumatic injury were transfused (57 minutes with transfusion versus 35 minutes without transfusion, p = 0,03). Conclusion: concerned population by pre-hospital transfusion is young and mainly male, affected by a traumatic injury threatening the vital prognosis in majority of cases. Despite the fact pre-hospital transfusion allowing survival, it should not delay patient transportation to allow hemorrhage final control.
