Contrôle de l'hyperdivergence en orthodontie

Abstract : Hyperdivergence constitutes a skeletal dysmorphosis of the vertical dimension. An extraoral, intraoral and functional clinical examination with cephalometric analysis on lateral teleradiography are essential to make a diagnosis. Hyperdivergence is both a cause and consequence of many dysfunctions, patients must therefore be referred to an ORL specialist very quickly so as not to aggravate the dysmorphosis during growth. Hyperdivergent patients are more difficult for orthodontists to treat than hypodivergent patients, their prognosis are usually less favorable. The mechanics that we use for braces treatment are indeed said to be “egressive”. Vertical dimension control is therefore essential and shall be carried out in the early stages of treatment. Parasitic molar egression can be achieved by wearing a high Pull face bow or by the use of miniscrews : these ones are increasingly being used because they are more effective and better accepted by our patients. Temporary anchorage devices such as mini-anchoring plates have shown good results in hyperdivergent subjects. However, in case of more severe dysmorphosis coupled with other anomalies of the anteroposterior and transverse dimension, orthognathic surgery remains the most reliable solution over time.
