Retour d’expérience des femmes en ce qui concerne le post-partum et la consultation post-natale : étude qualitative réalisée dans les Alpes-Maritimes

Abstract : Introduction: in France, pregnancy monitoring is a highly codified activity comprising seven consultations and three mandatory prenatal ultrasounds as well as a postnatal consultation. The sixth report of the National Confidential Inquiry into Maternal Deaths 2013-2015 revealed that cardiovascular diseases and suicides are currently the two leading causes of maternal mortality, reinforcing the notion that the health of pregnant and postnatal women is not limited to the obstetrics domain. The objective of this study is to explore the experiences, attitudes, and expectations of women regarding the postpartum period, specifically the fourth trimester of pregnancy.
Methods: we carried out a descriptive qualitative study using data extracted from focus group discussions with women residing in the Alpes-Maritimes who gave birth between January 1, 2021 and June 30, 2021.
Results: seven women were interviewed during three group discussions and one individual interview. The health determinants of postpartum women were found to be complex and dynamic, involving the relationship with the self, the development of the maternal bond, and the redefinition of the couple, in interaction with the relational context and characteristics of the environment. These encompassed both physical and mental health, including fundamental changes marked by fatigue, mood fluctuations, emotional lability, wavering self-esteem, feeling of loneliness, and a sensation of emptiness. These feelings could further oscillate between manic states of joy and disillusionment. These findings are in line with the psychological event of an identity crisis.
Conclusion: support for postpartum women deserves to be rethought so as to respond as closely as possible to maternal concerns. We propose the extension of such support for the duration of a minimum of one year after childbirth, and also include the father.
