Visite pré anesthésique du patient hospitalisé en France en 2018 : enquête de pratiques auprès des professionnels

Abstract : Background : The pre-anaesthetic consultation and visit are a French particularity in terms of anaesthetic safety. Since the 1994 decree, no study has evaluated the practice of the pre-anaesthetic visit (PAV). The aim of this survey is to assess the performance of this procedure in France (excluding outpatients) in 2018. Materials and methods : The Anesthesia Department of the University Hospital of Saint-Pierre (Reunion Island) distributed a questionnaire to evaluate practice concerning inpatient PAV (23 single- or multiple-choice questions), from June to December 2018 on the SFAR website. We identified and analyzed all questionnaires. Results : 1217 out of 1341 participants (90.8%) reported performing PAVs. 84.1% of the practitioners surveyed performed them the day before the procedure, 15.9% the day of the procedure, with less than 10 PAVs per day for 64% of them. For 57.3% of participants, the average time to perform a PAV was less than 5 minutes. When physicians sometimes did not perform pre-anesthesia visits, as was the case for 42.9% of them, lack of time was the main reason given (69.7%). For 83.9% of practitioners, the anesthetist who performs the PAV is not necessarily the one responsible for the patient's general anesthesia. 68.7% of anesthesiologists believe that PAV has a medical and legal interest. 56.9% thought that it was of interest "only for certain patients", unanimously for ASA 3-4 patients. Conclusion : Since 1994, the perioperative management of the patient has evolved considerably. It is therefore possible to re-evaluate the practice of pre-anaesthetic visit.
