Bruxism and its involvement in the alveolar bone: systematic review of the literature

Bruxism is a widespread phenomenon that can be characterized by inter-arch dental contacts. The forces exerted by a bruxing patient can be up to three times greater than the functional activity of the manducatory apparatus and will be transmitted to the teeth and periodontium. This work was particularly interested in the alveolar bone and the susceptibility of these loads to induce an architectural modification of the bone tissue. This phenomenon was stated by Wolff in 1892, who mentioned that depending on the loads to which it is subjected, bone is able to adapt its external cortical and trabecular structure. It was therefore reasonable to think that the phenomenon of bruxism with its consequences on the periodontium could cause a bone modification. The objective of this work is to highlight the impact of bruxism on the alveolar bone.
Le bruxisme est un phénomène répandu qui peut se caractériser par des contacts dentaires inter-arcades. Les forces exercées par un patient bruxeur peuvent être jusqu’à trois fois supérieure à l’activité fonctionnelle de l’appareil manducateur et vont être transmises aux dents ainsi qu’au parodonte. Ce travail s’est intéressé plus particulièrement à l’os alvéolaire et à la susceptibilité qu’ont ces charges d’induire une modification architecturale du tissu osseux. Ce phénomène a été énoncé par Wolff en 1892, qui mentionne qu’en fonction des charges auxquelles il est soumis, l’os est capable d’adapter sa corticale externe et sa structure trabéculaire. Il était donc raisonnable de penser que le phénomène de bruxisme avec les conséquences qu’il engendre sur le parodonte pouvait occasionner une modification osseuse. Ce travail a donc pour objectif de mettre en évidence l’incidence du bruxisme sur l’os alvéolaire.
