Le suivi médical des femmes durant le premier mois du post-partum : étude observationnelle menée auprès de 352 usagères

Introduction: the length of hospitalization in maternity wards continues to decrease. Postpartum care services are becoming increasingly available, particularly for the detection of postpartum depression, which affects 13% of mothers. The aim of this study was to assess maternal satisfaction with their medical follow-up during the first month after delivery.
Method: a quantitative observational study was conducted among 352 women with low medical, psychological, and social risk who had given birth by vaginal delivery and had a standard length of stay in a maternity hospital. They were recruited via social networks and interviewed using an online questionnaire.
Results: the overall satisfaction of the women was on average 6.5/10. Close medical follow-up was provided during the first days after the return home, mainly by a private midwife, but was irregular or non-existent afterwards. Almost one woman out of ten had no follow-up during the first month after the maternity stay. There was a clear lack of information in the antenatal period and during the maternity stay concerning medical follow-up of women after delivery, the maternal psychological state in the postpartum period and the frequent difficulties encountered during the postpartum period.
Discussion: an irregular follow-up during the first weeks after the return home and its non-binding status do not allow an optimal support of the women in this period.
Conclusion: to encourage a semi-global accompaniment of women by a referent health professional would allow to better anticipate the organization of their medical follow-up after childbirth and especially, to improve the detection of maternal psychological disorders.
Introduction : la durée d’hospitalisation en maternité ne cesse de diminuer. L’offre de soins en post-partum tend à se développer, notamment pour dépister la dépression du post-partum qui toucherait 13% des mères. Cette étude avait pour but d’évaluer la satisfaction maternelle concernant leur suivi médical durant le premier mois après l’accouchement.
Méthode : une étude quantitative observationnelle a été menée auprès de 352 femmes à bas risque médico-psycho-social, ayant accouché par voie basse et bénéficié d’une durée de séjour standard en maternité. Elles ont été recrutées via les réseaux sociaux et interrogées grâce à un questionnaire en ligne.
Résultats : la satisfaction globale des femmes était en moyenne de 6,5/10. Un suivi médical rapproché avait été assuré les premiers jours suivant le retour à domicile, essentiellement par une sage-femme libérale, mais il était irrégulier voire inexistant ensuite. Presque une femme sur dix n’avait bénéficié d’aucun suivi le premier mois après le séjour à la maternité. Un défaut d’information est flagrant en anténatal et pendant le séjour en maternité concernant le suivi médical maternel après l’accouchement, l’état psychologique maternel dans le post-partum et les difficultés fréquentes rencontrées dans le post-partum.
Discussion : un suivi irrégulier les premières semaines suivant le retour au domicile et son caractère non obligatoire ne permet pas un accompagnement optimal des femmes dans cette période.
Conclusion : favoriser un accompagnement semi-global des femmes par un professionnel de santé référent permettrait d’anticiper l’organisation de leur suivi médical après l’accouchement et d’améliorer notamment le dépistage des troubles psychiques maternels.
