Déclenchement artificiel du travail à terme à Port-Royal : Évaluation des pratiques professionnelles entre 1999 et 2009 en regard des recommandations de la HAS de 2008

Abstract : Term labor induction concerns, since 1995, one in five women. Medical or elective induction, this obstetrical practice should be evaluate regularly. The latest recommendations were made by HAS in 2008. What about Port-Royal practice's evolution ? Are the recommendations respected ? To answer this questions, we leaded a retrospective research, comparative to that made earlier by Brochard in the same maternity, including all low risk patients who had labor induction between July 1 and December 31, 2009 in Port-Royal. The main objective was to identify and assess the practice of term labor induction in Port-Royal and to compare with Brochard's results and the 2008 recommendations. Our secondary objective was to compare maternal and neonatal morbidity with the general population in Port-Royal. 204 patients were included in the study. It shows that term labor induction doesn't increase instrumental delivery or immediate neonatal morbidity. Nevertheless, medical induction of labor doubles caesarean risk (RR = 2,03 and CI [1,67 ; 2,39]) and moderate post-partum hemorrhage (RR = 1,82 and CI [1,43 ; 2,21]). The recommended method for induction of labor, regarding Bishop score, isn't observed in almost one third of case (26,9 % in medical induction cases versus 30,9 % in elective induction cases). To conclude, medical induction of labor is a good alternative to caesarean section. Nevertheless, professionals must ensure the best compliance with recommendations, to be the least possible iatrogenic, especially in case of elective induction.
