Comparaison des intoxications médicamenteuses volontaires entre l'île de la Réunion et la métropole

Abstract : Introduction: Self-administered drug overdoses (SADO) are a problem of public health. The profile of drugs used for SADO in Reunion Island, a French territory far from France, is it different? Method: Review of all SADO episods emergency room visits over a 1-year period during 2001-2002 in Tenon Hospital (Paris XXth) (M) and 2002-2003 in G.H.S.R (Reunion) (R). The drugs used were identified. Results : We compared 830 SADO episods and 1535 medicines in M with 574 SADO and 1008 medicines in R. Patient characteristics did not differ significantly during the two period (predominance of women 66,3 vs 69,2%;mean age:36,9 ± 13 vs 34,3 ± 13,7 years). Psychotropic drugs predominated in M (77,5% vs 52%). Those therapeutic classes were significatively more important in R: the analgesics (10,2% vs 6,8%), anti-inflammatories (6,3% vs 2,7%), cardiovascular drugs (5,2% vs 1,9%), anti infectious, herbal medicines, endocrinologies drugs (4,1% vs 0,8%). The psychotropic profile's was different for benzodiazepine drugs and antidepressants. The serotonin selective re-uptake inhibitors (SSRI) were increased instead of Imipraminic drugs in R, as the same evolution in Metropole. Among the analgesics, paracetamol alone predominated in R (87,4% vs 63,8%; p<0,01), but use of opiate analgesics was less frequent (7,8% vs 21,9%; p<0,01). The cardiovascular drugs, more important in R, presented no difference in their profile. Oral antidiabetic medications were more used in R (3,4% vs 0,3%; p<0, 01). Conclusion: Drugs used in acute self-poisoning are a mirror of medical prescriptions and the health of a population. Actually, cardiovascular diseases and diabetes mellitus are more frequent and the herbal medicine is more important in the local pharmacology. Trends in substances used for SADO is the same in the both centers. The study of the drugs epidemiology's is indispensable to adaptate our knowledge in toxicology and the measures of public health.
