Comprendre pour mieux agir‎ : étude descriptive d'une cohorte de 30 patients infectés par le VIH et "perdus de vue" à l'hôpital de jour de Cayenne

Abstract : French Guiana is the French overseas territory most affected by the HIV epidemic. This epidemic spreads in a territory that is marked by specificities impacting the follow-up of HIV patients. Studies and local epidemiologic data underline the importance of patients who have interrupted their follow-up. At the moment, there is no strategy to deal with this problem. The literature review shows that follow-up interruption is a contextual phenomenon. Inspired from the method of research-action, we conducted a preliminary study on 30 patients that had been lost to follow-up by the day hospital of Cayenne general hospital. The objectives were to acquire preliminary knowledge to design and implement a public health action aiming to find the patients that had interrupted their follow up throughout French Guiana, to understand the perception of these patients in order to identify risk factors, and to evaluate the use of the telephone as a way to recontact patients. In addition to the insights on the quality of life of patients, the present work underlines their vulnerability and the multiplicity of reasons why patients interrupt their follow-up. It also confirms the pertinence of telephone recall of patients having interrupted their follow up. Finally, sharing the experiences of these 30 life stories, allows to reconsider the offer of care in a new light and to identify opportunities. To improve the relation between patient and physician and to reinforce care networks around these vulnerable patients seem to be preconditions of a sustained follow up.
